建立居民健康档案实际操作现状，按照规范要求检查项目，血压、血糖、尿常规、体温、身高，体重、血常规、脉率、生化、心电图、中医体质等。现阶段各乡镇卫生院建立档案的流程:现场体检--手写纸质档案--回卫生院(街道卫生站)手动录入至电脑--上传国家数据库。手工建立居民健康档案效率极低，出错率高，需投入大量人力物力财力，纸质档案保存也造成大量档案管理成本，同时建党工作量大，重复操作多，建档效率低，很难保证建档数据的真实性。
